Look for the LOGIN TO MY ACCOUNT choice under the MY ACCOUNT menu on the top of the page. (Do not select Create an Account as your account is already in our system.)

Select RESET MY PASSWORD and on the following page you will be prompted to enter your email address. Enter the email address you use for your Serve You Rx Vermont account. A verification code will be sent to that email address. Go to your email inbox and open the email you receive from Serve You Rx Vermont.

In that email will be a link to reset your password. Simply click on that link and when our web page opens, you will be prompted to enter a new password. That's it!

(If you see a message telling you your username can't be found (or similar) then you have two options: You can either select NEW PATIENT INFORMATION FORM to create a new online account or contact us and we'll set up a new login for you.)

We've made a number of changes to serve you better.

If you're an existing patient, welcome! All you need to do is login to your account and you're ready to go. New patients, that is if you've never visited Serve You Rx Vermont before will need to create an account. To do so choose My Account from the top menu and select New Patient Information Form. Simply fill in the blanks to establish your online account and, after responding to a verification email, you'll be able to login and refill your prescriptions. (If you're already in our system you do not need to create a new Patient Informaton Form.)

(We're sorry but Serve You Rx Vermont is only available to members of organizations or employees of companies that have contracted with us to provide a pharmacy benefit. If your organization or company is listed in our New Patient Information Form you may create an online account with us. Please call us if unsure.)

Once you've established your account and logged into the site you can select My Account, then My Information if you ever want or need to change your account information. This includes your email address, mailing address and other contact information, as well as add or delete family members or dependents.

You can also Transfer Prescriptions under My Account as well as Refill existing prescriptions.

New! Automatic Prescription Refills: On average, patients are five days late refilling maintenance medications. Simply let us know if you'd like us to add you to our auto-refill service and you'll never have to worry about running out of medication.

From the My Account menu you may:

When you click the Refills Online link below you must provide a Rx Number, the Patient Last Name, Phone Number and Patient Date of Birth to complete an online refill request. You must also do this if you would simply like to review your prescription history and refills available. Optionally, you may provide an email address for notification purposes. You will then be presented with a Terms of Use policy you must agree to in order to continue using the site. (Please note: Until we have at least one of your prescriptions in our system and fulfilled you will not be able to view your prescription history, status or enter refills. The history shown only includes prescriptions active in our system. If you need more detail or payment history please contact us and we'll be happy to provide it.)

The next screen will display your Rx History listing available refills allowing you to select the refill you'd like to order. At the end of the page is a listing of refills no longer available. (If you need to have one of those refills renewed please select them as-needed and we will contact your doctor.)

The next screen displays Delivery & Pickup choices. Pickup is NOT available. Your prescription(s) will be delivered by mail.

Finally, you will be presented with an order summary where you can review your order and enter any comments you'd like to make. Please review your order carefully before clicking the Submit button.

After clicking Submit, you will have the opportunity to print a receipt for your order. Please do so and keep it handy as it has an order confirmation number. At any time you may call us a toll free at 866-926-3627 for assistance or to speak to a pharmacist.

Please use the other menu choices to participate in the Autofill program or transfer other prescriptions.
